Dementia Management and Support
Our interdisciplinary care plan team members work with our residents and families to create a comfortable environment and
support for activities of daily living including task-segmentation, simple routines and visual cues to compensate for
diminishing mental processes.
Diabetes Management and Education
Our experienced nurses and registered dietician consultant provide help with all aspects of managing diabetes including:
blood glucose monitoring, insulin administration and tracking, exercise plan, meal plans, education and more.
Therapy and Restorative Nursing Programs
Professional therapy programs are individualized according to resident condition and are available when ordered by the
physician. When professional therapies are not indicated, Restorative nursing ambulation, dining and exercise programs may be
utilized to support the resident and maintain function and ability so that quality of life is maximized for our long-term care
population. Restorative programs are individualized in the areas of:
